I bought everything at Michael's (also Joann's have them too... basically any craft stores or floral supply stores).  I've seen the glass gems also at WalMart, Target, even Home Depot- look in the floral/home decor sections.  Magnets are available at Michaels in round discs or in long strips.  The more common gems are smaller, in many colors, but clear and large ones work best for this.

I used epoxy for glass to paper and superglue for paper to magnet.  I like epoxy because it is gel-like and has a thickness so you can squeeze out the air bubbles between the paper and glass.  A note about the paper, if you use very thin paper (like Japanese origami), you need to glue one more layer as a backing.  Otherwise, the magnet outline will show through the paper, you'll end up with a big black dot in the middle.
